Payment & Insurance

We are committed to providing you with excellent service in every area including billing and insurance.

Because billing and collection costs often lead to increased fees, it is our policy that known routine charges, such as non-covered services are paid at the time of service. Co-payments, co-insurance and deductibles will also be collected at the time of service. Copayments are typically listed on your insurance card and we do our best to contact your insurance company, prior to your scheduled appointment, to verify co-insurance and deductible amounts. So it is important that you provide us with complete and accurate insurance information at the time you schedule your appointment.

We offer many payment options. These include credit cards (MasterCard, Visa, Discover, American Express and Diners Club), debit cards, cash or checks. We will always be pleased to discuss our fees with you before we provide any services.

Insurance

We do recognize that understanding insurance is not always easy. The best way to find out exactly what coverage you have is to contact your insurance carrier or human resource director at the employer who administers your benefits. We can also provide resources to help you by calling our experienced insurance specialists at (513) 221-5274 extension 71. They would be happy to assist you in determining the exact coverage you have. They will need complete and accurate information from any medically-related or vision-related insurance cards.

Insurance policies and health plans are very complex and constantly changing. We would be delighted to file your insurance and Medicare claims for you. We simply ask that you provide us with complete and accurate insurance information. We will make every effort to process your claims as quickly as possible. Company policy is that you will not receive a bill from us until the insurance company has responded to your claim. Insurance companies can respond by: paying the entire claim, paying part of the claim, or denying the claim. Please keep in mind that you are billed for any amounts not covered by your insurance plan. We appreciate your prompt attention to these balances and expect payment upon receipt of out billing statement.

We are Medicare “participating providers”, which means that our doctors agree to accept what Medicare allows. Of the allowable, Medicare typically pays 80%, and the patient will be responsible for the 20% balance. Medicare patients are also responsible for their annual Medicare deductible (2010 – $155.00) and all non-covered services (example – refraction).

We participate in most of the major insurance and managed care plans (example: Anthem, Aetna, Humana, United HealthCare, Medical Mutual plus many others). Our goal is to make our eye care services as affordable and convenient as possible. It is best to contact the office or the billing office to verify if the physician participates with your particular benefit plan.

Each major insurance company tends to have many plans within the plan so it is extremely important to verify our participation as well as what benefits you have for service that our physicians would provide. Your coverage is determined through your contractual agreement with the insurance company and your employer and is ultimately your responsibility to verify participation and coverage.
